Private Pay Services

Companion services $15 per hr:
Companion services consist of non-medical care, supervision and socialization/community inclusion activities provided to an adult to support the Consumer in daily valued routines in the community. A companion provider may also assist the Consumer with such tasks, as self-care needs, meal preparation, laundry and shopping; however, these activities shall not be preformed as discrete services. This service does not entail hands-on medical care. Providers may also perform light housekeeping tasks, incidental to the care and supervision of the Consumer. The service provides access to community-based activities and should be defined as activities most likely to result in increased ability to access community resources. Companion services may be scheduled on a regular, long-term basis.

The responsibilities of a Companion include but are not limited to:
  • Providing companionship and conversation
  • Providing stabilization and assistance with walking
  • Prepare grocery list, organize mail, check food expiration's
  • Providing medication reminders
  • Assisting with self-care needs
  • Assisting with laundry
  • Assisting with shopping/errands
  • Providing transportation to community-based activities/resources
  • Non-Medical care
Personal Care Assistance $18 per hr:
Assistance with eating, meal preparation, bathing, dressing, personal hygiene, and activities of daily living. Also includes light housekeeping when these activities are essential to the health, safety, and welfare of the Consumer and when no one else is available to perform them. Personal Care Assistance may not be used solely for supervision.

The responsibilities of a Personal Care Assistant include but are not limited to:
  • Assist with eating
  • Assist with meal preparation
  • Assist with bathing
  • Assist with dressing
  • Assist with personal hygiene
  • Assist with activities of daily living
  • Light housekeeping essential to the health, safety and welfare of the client
  • Lifting

Private Duty Nursing Services
(RN - $40 per hr)
(LPN - $30 per hr)
(CNA - $20 per hr)
Private duty nursing service are prescribed by a physician and consist of individual, continuous nursing care provided by registered or licensed practical nurses. must provide provide duty nursing services in accordance with Chapter 464, F.S. Limited to service not otherwise covered by Medicaid State Plan.

The responsibilities of a Private Duty Nurse include but are not limited to:
  • Performing care as prescribed by a physician
  • Individual continuous nursing care
  • Part-time or intermittent care
  • Provide private duty nursing services in accordance with Chapter 464, F.S.
Respite Care Provider:
$40 per hr RN / $30 per hr LPN / $20 per hr CNA

The responsibilities of a Respite Care Provider include but are not limited to:
  • Providing supportive care and supervision when the primary care giver is unable to perform these duties due to:    
              - Planned brief absence
              - Caregiver is available but temporarily unable to care for or supervise the client for a                        brief period of time
  • Provide the normal caregiver(s) a break from the normal caregiver routine
Mileage at a rate of $0.57 will apply if the caregiver is using their own personal vehicle

Transportation services are included in some our services. Transportation is provided but not limited to:
  • Community-Based Services
  • Doctor appointments
  • Dental appointments
  • Errand/Shopping
